Please read carefully before proceeding
- Students who wish to receive their master diploma are required to submit the graduation form.
- In case the master programme includes a final thesis, the student is also required to upload their thesis to the Leiden Repository. The CSM Portfolio should not be uploaded to the Repository.
Rules regarding the completion of the form
All questions in the form should be answered fully. The Educational Service Centre (Onderwijs Service Centrum, OSC) checks whether you have met all requirements to graduate. If you have any questions about completing the form or about your eligibility for graduation, you can contact the OSC via osc.fgga@fgga.leidenuniv.nl. Please keep an eye on your uMail inbox in case the OSC has questions about your application.
Deadlines
There are two graduation periods each year; January with resit or extension in February/March, and June with resit or extension in July/August. The graduation date (= the date on your diploma) will be the final business day of the month in which you have obtained your final ECTS. File your graduation application as soon as you know you are ready to graduate, but preferably no later than the final business day of that month. The OSC needs approx. 4-6 weeks to process each graduation application. Please note that during busy periods like the summer months processing can take longer than usual. If you wait too long before filing your application we can no longer guarantee your diploma will be ready in time for the graduation ceremony. The graduation ceremonies take place twice a year, in the spring and in the fall.
Check your results and requirements
Before you send in your graduation application, please check via Course History and Academic Requirements in uSis whether all credits you need are registered, with the exception of the thesis/portfolio. Your thesis/portfolio grade and credits will be registered by the OSC during the processing of your application. If you have passed (a) course(s) that is/are still missing in MyStudymap or uSis, please let the OSC know which course(s) is/are still missing and when you passed it/them.

De-registering via Studielink
Once you have completed your programme, please make sure you end your registration in Studielink per the 1st of the next month, citing ‘graduation’ as the reason for your de-registration. |

Consent form for the publication of a thesis
|
The Leiden Repository is an online archive that makes the MSc theses of Leiden University programmes easily searchable and publicly available (if desired). For more information, see this page of the repository. MSc theses are made publicly available unless supervisor and student decide to place the thesis under a temporary or permanent embargo.
The OSC uses this Consent Form to check if the information you have filled out for your repository upload is correct, so the upload can be processed. Please note that it may take up to a few months before your thesis is entered into the repository.

Non-plagiarism and publication statement
- I declare that the digital version of my MSc thesis I have uploaded to the Leiden Repository is the same as the version I have handed in to my supervisor for the plagiarism check.
