** Voor de opleiding Media Technology worden geen vrijstellingen ingewilligd. U kunt een verzoek indienen voor een vakvervanging.

Voor elk vrijstellingsverzoek moet een apart formulier worden ingediend.
Gelieve dus één vrijstelling per formulier aan te vragen.
Let op dat het vak dat u gebruikt als basis voor de vrijstelling minimaal het aantal EC moet hebben als het vaak waar u vrijstelling voor aanvraagt.

Let op! Voor verzoeken o.b.v. de 'laatste kans regeling' moet uit uSis blijken dat het vak waarvoor een extra kans wordt aangevraagd daadwerkelijk het enige nog af te ronden vak is.
Please note! For petitions based on the 'laatste kans regeling' it must be evident from uSis that the course for which the petition is filed is the only course left to complete the study program.
Toelichting:
Voor het beoordelen van een keuzevak heeft de examencommissie een beschrijving van het vak nodig, die als PDF wordt meegezonden bij dit verzoekformulier. De examencommissie beoordeelt de invulling van de keuzeruimte als geheel, waarbij wordt gelet op aantal EC, niveau en samenhang. Het is dan ook van belang dat alle vakken (30 EC) worden ingevuld.

Voor minoren buiten Universiteit Leiden, TU-Delft of Erasmus Rotterdam is toestemming van de examencommissie nodig.
Wijzigingen: indien u één (of meer) vak(ken) van een minor niet haalt, kunt u een verzoek indienen waarbij u de niet behaalde vakken vervangt door andere keuzevakken. U dient dan een verzoek in tot goedkeuren van een keuzevakkenpakket. Vul hier wederom alle vakken van het keuzevakkenpakket in.

Let op: toestemming van de examencommissie is geen garantie op toelating tot de vak(ken).
Met dit formulier vraagt u toestemming om de vakken op te nemen op uw diploma. Inschrijven/registratie geschiedt in uSis.

Explanation
For your petition to be processed by the Board of Examiners LIACS, you must upload a course description as a PDF-file.

For Minors taught outside of Leiden University, Delft University of Technology, or the Erasmus University Rotterdam, you will need approval from the Board of Examiners. The Board will consider the number of EC, level of the courses, and the overal cohesion of the electives.
Adjustments: in case you did not finish one (or more) of the approved courses, you will need to file a new petition in which these courses are replaced by different elective courses. You will file a petition for the approval of elective courses.

Please note: approval from the Board does not guarantee admission to the course(s).
By filling in this form, you request permission to include these courses in your study programme and on your diploma. Course enrolment is registered in uSis.
